I hereby grant Beyond Pink Foundation (“Foundation”) and those acting in pursuant to its
authority, full permission that is irrevocable, perpetual, and unrestricted for the use and
reproduction of images, including but not limited to photographs, videos, and recordings,
as well as likeness and statements, verbal or written, of/from me or my family without
compensation or benefit. This authorization includes the right to edit, alter, copy, exhibit,
publish, or distribute such images, likeness, or statements for any lawful purpose.
In addition, I release the Foundation and those acting pursuant to its authority of
responsibility for any violation of personal or proprietary rights that I may have in
association with the use and reproduction of images, likeness, and statements.
Furthermore, I hereby hold harmless, release, and forever discharge the Foundation and
those acting pursuant to its authority from all claims, demands, and causes of action,
including claims of libel and invasion of privacy, which I, my heirs, representatives,
executors, administrators, or any other persons acting on my behalf or on behalf of my
estate have or may have by reason of this authorization.
Furthermore, this release does not infer any guarantee or obligation for the Foundation to
use and/or reproduce such images, likeness, or statements.
I have read and understand the above release. I affirm that I am at least 18
years of age, or, if I am under 18 years of age, I have obtained the required
consent of my parent(s)/guardian(s) as evidenced by their signatures below.
